Summary of the proposed change initiative

Patient education will form a significant aspect of the change strategy required to reduce the incidences of hospital-acquired infections (HAIS) among patients aged above 65 years. The initiative will follow a cyclic process, which will start by planning the change strategy. At the planning stage, the change management team will determine eh resources required to fulfill the project’s requirements. The required facilities will include human, financial, ad physical resources. A change implementation team will be developed comprising of members of a multidisciplinary team. The role of the implementation team will be to determine the activities that will be performed by all stakeholders to achieve the goals and objectives of the project.

Another significant role of the change implementation team is to develop evaluation tools to determine the program’s effectiveness (Umucu, 2020). The implementation process will involve the categorization of subjects into two groups; control and comparison. In one category, the clients will be engaged in a blind study where they will be trained on hand hygiene effectiveness and allowed to use water and soap to achieve this goal. The second category will include individuals using hand sanitizers instead of soap and water to achieve hand hygiene. Justification of change and Impacts on Targeted Population

HAIs are a significant public health concern contributing to negative clinical outcomes such as increased morbidity and mortality, prolonged hospital stay, and higher medical costs. Geriatric clients are at higher risk for HAIs due to their declining physical, immunological, and cognitive functions (Kukla et al. 2016). The change strategies support the targeted population by lowering the rates of HAIs and improving clinical outcomes. Both hand hygiene strategies, use of soap and water and hand sanitizers, effectively reduce HAIs. My role as part of the change implementation team will be to provide patient education and monitor the program’s implementation and evaluation phase.
