Some Concerns/ Questions as the MSN Program Commences

The delight that I have because I am finally commencing my journey to becoming a Family Nurse Practitioner (FNP) is immense and immeasurable. Like the other students, however, there is that lingering fear of the unknown which I believe is normal. The fear is brought about by the many questions that we have concerning the future as an advanced practice registered nurse or APRN. In this discussion, I present three of some of the questions I have after doing some research on APRN practice in the country.

  1. If I will practice in a full practice authority (FPA) state as an autonomous FNP, which malpractice insurance cover will be the best to choose between Occurrence Policy and Claims-Made Policy?

It is my wish to in future set up my practice in a state that allows autonomy in practice for APRNs. This will allow me to practice to the fullest extent of my knowledge and training. However, I have asked around and discovered that the autonomy that comes with full practice authority also places the APRN at considerable risk for malpractice suits from disgruntled patients and relatives of patients (Buppert, 2018). From my understanding, successful litigation of an APRN can spell doom financially and spoil their professional reputation irreparably. In my quest to find out more information about this, I have come to learn that there are malpractice coverage options for APRNs in practice in anticipation of this kind of scenario. The two plans I have come about are a Claims-Made Policy and an Occurrence Policy (Loria, 2017). My question, therefore, is which of these two malpractice policies is the best for a nurse practitioner practicing autonomously in a FPA state?  MSN Program Commences Essay.

  1. If I will practice in a reduced practice or restricted practice state with a collaborative agreement between me and a practising physician, will the malpractice cover of the physician cover me or will it be prudent for me to take an individual malpractice cover?

Still on the matter of malpractice, there is the possibility that I may alternatively get to practice in a state with no policy for autonomous practice for APRNs. In these states with reduced practice or restricted practice, I will be forced to have a collaborative agreement with a practising physician who will supervise my work and whom I will work under. This is not what I wish for, but it is a reality we will have to contend with just like those who have come before us. My question on this, therefore, is whether it will be prudent for me in this situation to take an individual malpractice cover or to remain under the collective cover (if there is anything like that!) with the physician?

  • Why is it that there is no equitable reimbursement for APRNs and physicians who have performed the same procedures in many states?

My last concern is the matter of equitable pay for APRNs and physicians with regard to similar procedures done by both of them. I have learnt that Medicare reimburses APRNs at 85% of the physician rates for similar procedures done. I have also learnt that Medicaid leaves this decision to individual states, who in their wisdom reimburse APRNs at between 75% and 100% of the prevailing physician rates for similar procedures in the state concerned (ThriveAP, 2016). My question, therefore, is whether there are any policy efforts at the federal level to level the reimbursement field for APRNs who perform the same procedures as physicians? I have learnt that some procedures and treatments by APRNS at times even achieve better patient outcomes.
