Executive Summary

Healthcare systems dedicated to improving care, reducing expenses as well as enhancing patient experience face numerous challenges such as the need to align changes occurring at various levels of the organization (Manalili & Santana, 2020). However, the process of prioritizing, identifying, and implementing the various changes can be enhanced with the right process, tools, and people. Ideally, once processes, tools, and people are in alignment, the healthcare system can address clinical, operational, and financial quality improvement programs and projects such as pharmacist-steered medication therapy management used to reduce the total cost of care (Manalili & Santana, 2020).  typically, the quality improvement initiative can help achieve tremendous strides in the financial, clinical, and operational health of the hospital.

Almost half of all Americans use at least one prescribed drug while others use more than one drug thus indicating that prescription medication use is commonly used and is increasingly complex (Hines et al., 2020).  Unfortunately, high prescription usage results in medication-related challenges.

According to research, more than 1.5 million preventable all mediation-associated severe events take place each year which incurs a cost of more than $177 billion in medication –associated mortality and morbidity (Coe et al., 2020). MTM is a quality improvement initiative that can be used to improve patient outcomes while reducing costs in healthcare.  The initiative will be used to expand the involvement of pharmacists performing medication therapy management to a group of patients who use a shared-risk contract. Conceptually, MTM pharmacists not only reduce the total cost of care on healthcare organizations but for patients too. the MTM will make pharmacy leaders integrate the data showing the positive impact pharmacists have on patient outcomes (Pestka et al., 2020).  It is important to note that MTM therapy is a service offered by pharmacists to eligible patients who have multiple diseases and as a result, leads to several medication-related challenges. Essentially, MTM helps optimize the therapeutic results and outcomes of the patient and assist detect and prevent expensive medication associated with severe events (Pestka et al., 2020). Therefore, after a thorough and comprehensive review of the medications including the side effects and financial barriers and the functionality of certain medications, pharmacists can assist their patients to develop action plans to enable them to get the most out of their prescriptions. Further, pharmacists collaborate with a patient’s healthcare givers to optimize the use of medication for improved health outcomes.

To successfully, implement the initiative, it will require collaboration from everyone in the healthcare system. Therefore, doctors, nurses, administrators, and data analysts.
