Please rephrase the second outcome- see below (assignment part 1) how I rephrase it. The goal is more identification of adolescents who are suffering from depression then needs referrals.

Anticipated Changes
In light of the aforementioned planned project, the project manager expects the following positive changes: First, the number of PHQ-9 screenings completed during adolescent wellness visits should rise after educational implementation. According to Beck et al. (2021), adequate provider knowledge regarding depression screening tools is crucial for the effective identification and management of depression in adolescents. The improvement will help to establish a more accurate and effective identification of depression signs in adolescent patients. Data Collection Methods
For each of the outlined outcomes, the following methods will be used to collect data: To assess the result of outcomes regarding correctly completed PHQ-9 screenings during wellness visits, the office manager will review the patient medical record notes and visit summaries within the clinic’s EHR system. The manager will remove the personal identifiers from the records to maintain patients’ privacy but give the correct report on screening completion on a daily basis. For the outcome of patients who screen positive and need referrals to mental health services, the manager will follow the records in the EHR system, using the de-identified data to establish the number of adolescent patients who screened positive and referred for further evaluation of their mental health.
Finally, to assess the provider’s knowledge, the project manager will employ pre and post-test questionnaires formulated from the educational information that will be given during the training sessions. These surveys will be adapted from validated instruments used in previous studies on depression screening education, such as the Knowledge, Attitudes, and Practices (KAP) survey. Andrade et al. (2020) note that the KAP approach helps guarantee a proper collection of data besides following the required ethical considerations towards the participants to ensure adequate and ethical data collection and participant protection. To ensure anonymity, each participant will be assigned a unique code known only to the participant and the project manager. This code will be used to match pre-test and post-test responses without revealing the participant’s identity. The questionnaires will be in paper format, with participants entering their unique code at the start of each test.
