My preceptor and I attended and participated in the monthly clinical practice council nursing meeting. In the meeting, we discussed the various topic to help maintain the hospital magnet status such as to monitor changes in practice for deviations from standards and make recommendations for corrective action. Also,

to keep abreast of current evidence as it pertains to clinical practice and spearhead practice changes in response to new evidence. We also discuss how to evaluate current clinical practice, identify areas of deficiency and delegate these items to the appropriate Practice Committee for evaluation and resolution. On this day, my preceptor and I attended several meetings to discuss the newly implemented electronic health record (EHRs) system. The hospital recently transitioned to a new EHRs but the Go-live was not very successful and there are a lot of important area of documentation which did not carry over to the new system. Today we met with the radiology department to discuss their concerns with specimens’ collection workflow.

Today my preceptor and I met with the Super-Users to discuss some of the concerns they have noticed in the various hospital units they had been assigned to and what the staff has complained about since the newly electronic health record (EHRs) system has been implemented. In the afternoon we also met with the representatives of the OBGYN department to discuss and resolve some of the major concerns they have also noticed since the Go-Live date of the new EHRs. One of the major issue is the staff inability to document a scheduled C-section, as well as the staff inability to drag/drop scheduled cases as they were missing “diagnosis.”

Today, my preceptor and I continued to meet with the representatives of the hospital various units to discuss and resolve some of the issues their units have encountered since the Go-Live date of the newly implemented electronic health record system. Today, my preceptor and I attended various meeting regarding the newly implemented electronic health record (EHRs) system. We met with the representatives of Epic EHRs to discuss all the tickets that were created since the Go-Live, and to come up with a plan to fix all these issues that had been encountered. We also met with the Periop to discuss the issue they have with the patients’ dropping off the big board when they go to surgery.

Today, my preceptor and I attend a meeting regarding the center for Medicare and Medicaid services (CMS) survey. During the all-day long meeting, we went over the HCAHPS Domains which included the nurse communication, doctor communication, responsiveness of hospital staff, cleanliness and quietness of the hospital environment, pain management, communication about medications, discharge information, overall rating, care transitions and likelihood to recommend.

The practicum site is swiftly embracing the new technology for glycemic glucose control as in the preceptor who is the primary educator on this project. The glucometer project is resumption from the last quarter practicum hours in which the contracts for product purchase and implementation negation were the primary activities. The first few classes that have occurred with the nursing super-users have been relatively successful but are significantly overshadowed by the current status of the nursing workforce. The practicum organization is currently short nearly 100 nurses from the 500 nurses normally employed in the acute care unit. To address this, nurses are being forced into mandatory overtime shifts every week in addition to the required training for this new glycemic software. When consulting leadership and reviewing the turnover numbers and factors within the organization, it is apparent that the practicum site is not experiencing a trend that is not occurring at the national level. The current nursing turnover rate in the United States is 16.5% (Mazurenko, Gupta and Shan, 2015). The practicum site turnover rates at under 15% at last count, however, I am working with leadership and Human Resources to provide a more accurate, updated number from June of 2015. Further, Hill et al. (2015), reported some key innovative towards preserving current staffing resources at the system level to elevate some of the hardship related to the nursing shortage. Efficiency tools such as daily huddles and down-staffing in four hours increment, a centralizing monitoring system (Teletracking) to ascertain the patient census and staff accordingly are currently in use by the organization. Much of the last three weeks have been spent collecting information from staff after their scheduled training  session for glucometer project. It is apparent that these nurses are tired, are scared about the future state and are clearly missing a confiding mentor to unburden themselves and offer suggestions for improvement.
