Nursing as a professional discipline enables the nurse to encounter people from diverse cultural backgrounds. These are patients and their relatives who also hold divergent opinions, beliefs, and convictions about health and illness. The advanced practice nurse is a specialist who is educated and trained in the assessment, management, and follow-up of patients from a particular population focus. As a specialist, the APRN follows strict standards of practice and uses evidence-based practice to restore health and promote wellness. However, there will be times when their professional philosophy and worldview is challenged by or clashes with that of a patient or their next of kin. A Personal Worldview that Most Influences Personal Philosophy of Care and Practice

My personal worldview as a healthcare professional is a theist worldview. To be specific, it is the Christian worldview or perspective (Reimer‐Kirkham, 2019; Summit Ministries, 2019). This is the worldview that most influences my personal philosophy of and attitudes towards patient care. It also influences my personal philosophy of practice as an advanced practice registered nurse (APRN). To begin with, I subscribe to this worldview because I believe that there is a higher deity or God who has power over disease and illness. This belief is not contradictory to my calling as a nurse, as one may ask why I treat patients if the deity is able to heal just by believing. The explanation is that the Christian worldview I hold is cognizant and appreciative of medical science as a tool to help humanity. It recognizes that healthcare workers (physicians and nurses) just treat but have absolutely no power to heal. This power rests with God who is all-powerful (omnipotent), all-present or omniscient, and forgiving. This is the religious aspect of my Christian worldview (De la Cruz & Robinson, 2017). Spiritually, I believe that if the patient believes in God their body will react positively and receptively to medications and other measures that will then enable them to be cured or healed. Culturally, my personal worldview teaches me to “love my neighbor as I love myself”. For this reason, I have the philosophy of respecting each and every culture that a patient may present with. I see all my patients as the proverbial ‘neighbor’ in biblical teachings. This worldview in its three facets (religious, cultural, and spiritual) therefore influences how I view my APRN practice and patient care. Nursing Theory Most In Line with My Personal Philosophy and Approach to Patient Care

The nursing theory that I find most in line with and complementary to my personal philosophy is the Transcultural Nursing Theory or TCN put forward by the nursing theorist Madeleine Leininger (McFarland & Wehbe-Alamah, 2019; Sagar & Sagar, 2018; Sagar, 2015). According to this nursing theory, nurses are supposed to be able to work anywhere in the world regardless of the culture of the community in which they will be working. In other words, there is nothing like cultural shock for a nurse. A nurse is expected, according to this theory, to be culturally receptive and competent to take care of any patient who may find themselves in the care of the nurse and with a different set of beliefs than those of the nurse. The nurse is supposed to transcend cultures and accommodate all of them. They are expected to understand the patient’s preferences and beliefs by being empathetic. Above all, they are not supposed to challenge these beliefs or opinions just because they are different from their own. This way, it will be possible for the nurse to provide holistic patient care to any patient by being culturally sensitive and competent. Holistic care takes care of the physical, spiritual, emotional, psychological, and cultural.

There are several similarities between this theory and my personal philosophy of practice and approach to care. My theist Christian worldview prescribes love for the patients I care for, while the TCN theory suggests that the nurse be empathetic and refuse to be judgemental of other people’s cultures when they do not conform to their own. Transcultural nursing theory also requires the nurse to accept and compliment the beliefs of the patient. A challenge therefore comes when a patient comes and requests a nurse that they are allowed to continue taking their traditional herbal remedies from home to complement conventional treatment, or that they are allowed to keep traditional medicinal paraphernalia with them. Of course the nurse right away knows that these measures are not evidence-based and may actually be unhelpful. But this is where my personal philosophy or worldview as well as the TCN theory become helpful. “Loving my neighbor as I love myself” will man that I allow the patient what they believe is helpful to them and makes them happy, as long as I can reasonably deduce that they will not bring them any foreseeable harm. For TCN, it directs me to understand the patient’s culture and embrace it without despising it. This is being culturally competent and sensitive.
