In preventing patient harm, three areas did not meet the rating standards. In maternity care, the cesarean sections never met the required rating. The hospital recorded and increased the number of cesarean sections for first-time mothers even when there were no complications. This can be achieved through enhancing reproductive health education to mothers and the importance of natural birth. On medication safety two areas did not meet the set standards. One of the measures is medication reconciliation where the hospital displayed some levels of unintentional discrepancies per medication. The hospital should be able to reconcile and make documentation of any changes around medications and also clarify that all the medications prescribed and their dosages are appropriate. In total joint replacement, the two measures never met the required achievement. Moving forward there is a need to ensure proper protocols are put in place in ensuring total knee replacement surgery is done to only patients meeting the set criteria. Also, the surgeons must be given a chance to conduct at least 25 procedures annually to meet the required procedures. Additionally, the total hip replacement surgery measure only met a considerable achievement since it has not met the standard of 50 procedures annually. To advance from this MMMC (2022) the facility should ensure the surgeons meet the standards of conducting at least 25 procedures annually and also there is a need to put in place protocols that ensure only patients who meet the criteria are taken through this procedure.
