Measures of Quality: Falls

Nursing quality of care has typically been of principal focus in efforts to improve nursing care with regards to effects on nursing personnel, other medical personnel and patients. Nursing quality indicators are the elements of patient care that are directly influenced by nursing practice. Centers for Medicare and Medicaid Services (CMS) has presented as a no-pay policy to target patient falls in medical facilities. In 2008, CMS presented a policy that stopped reimbursing medical facilities for costs incurred related to falls among patients. To be more precise, the CMS does not reimburse medical facilities for traumatic injuries on patients from falls. The intention was to incentivize medical facilities to make greater use of fall prevention strategies. As a result of the policy, nursing care is making greater use of bed alarms, sitters, room changes and physical restraints as strategies for preventing falls (Fehlberg et al., 2017).

Nurse personnel have complex and potentially conflicting objectives when providing medical care to patients. They need to treat the medical condition that prompted the patient to visit the medical facility ad be admitted, keep the patient safe, and help the patient to recover or maintain mental and physical function. As such, there is a need to balance fall prevention against other priorities. Implementing falls prevention strategies has care quality, ethical and nursing practice implications. It has implications for care quality by ensuring that patients are kept safe in the medical facility so that their medical conditions do not worsen, rather they improve.
