Submission and Grading Information

The client XX, a 56-year-old Caucasian male had a diagnosis of type 1 bipolar disorder, with depression. The client had other medical conditions that include hypertension and hyperlipemia. During the presentation, the client reported symptoms such as persistent sadness, sleep disturbances, somatic leg pain, irritability, fatigue, and lack of appetite. He also reported that he feels that his family, especially his children have abandoned him. He also reported that sometimes he experiences mild maniac symptoms whereby he has high energy levels and feels overly excited. However, the client denied any suicidal thoughts and was willing to participate in psychotherapy. The client has been taking Seroquel 50mg at bedtime for bipolar type 1 disorder and Trazodone 100mg for the depressive symptoms and insomnia. The client added that he sometimes takes sleeping pills whenever he experiences an excessive lack of sleep.Discussion: Clinical Supervision Essay.
Therapeutic Approach with the Client
The existential-humanistic therapeutic approach would be useful for this client. Existential- humanistic therapy is an approach that focuses on the individual taking responsibility for their life and personal freedom (Varvatsoulias, 2015). Therefore, during therapy, the life purpose and reasons for the individual’s existence are discussed. It is also emphasized that everyone has the freedom to make decisions about their life. Therefore, the existential-humanistic therapy will be suitable for this client as he will be taught how to face the realities in his life and make changes where necessary, instead of focusing on the negatives. The client will also be advised to search for the purpose and meaning of his life and try to exercise self-awareness. Through this kind of therapy, the client will experience free thoughts and identify a way to handle his inner world and inner self (Varvatsoulias, 2015).Discussion: Clinical Supervision Essay.
As Wolfe (2016) explains, individuals with bipolar disorder or depression often have challenges in understanding and dealing with isolation since they believe that their loneliness and isolation are idiosyncratic and synthetic. This is evident for this client as he feels that his family members and children have abandoned him, which could be the major contributing factor to the depressive symptoms. Therefore, existential-humanistic therapy would be effective in helping the client to learn that loneliness and solitude are part of life and loneliness can be handled by investing the time in other activities and learning to cope and accept solitude. Additional Information about the Client
The client admitted to taking sleeping pills whenever he experiences excessive insomnia and thus, he is at risk of accidental overdose from the sleeping pills. It is important to educate the client about the risk associated with excessive intake of sleeping pills, the addictive aspect, as well as the synergistic effects of taking alcohol while taking sleeping pills (Guina&Merril, 2018). In order to reduce the risk of the client experiencing withdrawal symptoms, the client will be advised to withdraw from the sleeping pills gradually. The client reported feeling lonely and thus he will be advised to increase his social interactions, even with friends. The client also reported that he sometimes takes alcohol to stop feeling sad and thus alcohol cessation will need to be addressed in his treatment plan. His medications will also be reviewed to ensure assess for any response to the medications and examine if the client is experiencing any side effects.Discussion: Clinical Supervision Essay.
Recommendations
The client admitted that he was willing to make changes in his life and adhere to all prescribed treatment regiments. However, he was still blaming his loneliness and emotional problems on his family members. Lack of acceptance that his mental health is his responsibility maybe be a major obstacle to his recovery. Therefore, it is recommended that:
• The existential-humanistic therapyshould continue being used for this client to convince him that he is responsible for his life and therefore he needs to take charge of his recovery
• The PMHNP needs to perform a lot of verbal reinforcement in order to encourage the client to take responsibility for his recovery
• The therapist should help the client to sustain change by encouraging him to adhere to the prescribed treatment.
