Ethical decision making is not the sole domain of the physician. Nurses along with a patient care interdisciplinary team may be better prepared and have more opportunities to discuss moral problems with patients and families. Can you describe an instance in which the physician assumed the authority to make an ethical decision, denying nurses (and perhaps the patient and family) participation in the decision-making process? What are your thoughts on involving an interdisciplinary team? Does this team approach improve patient and family outcomes? If you feel you have an important contribution to make in a particular circumstance, and your opinion is not considered, how do you react? How do you think nurses can overcome the strong heritage of subjugation? Decision-making in Nursing Practice Essay.

Clinical Scenario Where A Physician Assumed Authority In Ethical Decision Making

While working in an inpatient oncology center, there was a 12-year-old male patient named Brian who was diagnosed with acute lymphocytic leukemia at 6 years.  After enduring multiple relapses that required several chemotherapy rounds with very shot remission periods, Brian was readmitted in the pediatric oncology unit after cancer metastasized to his brain and lungs.  During a meeting to discuss his options for treatment, Brian’s attending physician explained that since the cancer was progressing rapidly, and he was unresponsive to treatment, Brian felt that he had endured enough and did not want additional interventions and heroic measures. Therefore, he requested that a DNR order (do not resuscitate order) placed on his chart. However, Brian’s parents did not agree with the decision and course of action and preferred to hold on to hope for further treatment to prolong life.

Involving the Interdisciplinary Team

Saint-Pierre, Herskovic & Sepúlveda (2018) describe interdisciplinary teamwork is a complex process where different healthcare professionals work collaboratively to share knowledge, expertise, and skills to improve patient care. Interdisciplinary teams require confidence, competence, and commitment. They reduce the level of stress that patients can experience positive outcomes and for nurses, it decreases work-related burnout. Effective interdisciplinary teams have shared responsibilities with interdependence between roles and respect for the autonomy and experience of team members. According to Saint-Pierre, Herskovic & Sepúlveda (2018), involving an interdisciplinary team improves family and patient outcomes since it ensures that goals are clearer with shared roles and responsibilities. Interdisciplinary teams allow both patients and healthcare providers to participate in joint assessments, the diagnosis process, and in setting goals and recognizes the overlap that exists in the expertise and knowledge of staff from different disciplines. This ultimately encourages creativity and team cohesiveness through commitment and identifying mutual goals (Saint-Pierre, Herskovic & Sepúlveda, 2018).

Overcoming the Heritage of Subjugation

Nurses can overcome the heritage of subjugation through empowerment, mindfulness, and communication. Empowerment creates an opportunity for professional and personal growth, which enhances nurses’ ability to be included in decision-making processes with equality as a major principle that helps nurses to realize collective and individual strengths and abilities to make changes in public and personal lives (Molina-Mula et al., 2017). Mindfulness is a significant tool that nurses can use to act on their thoughts and emotions. Communication is a skill that helps nurses to become more assertive without being destructive. Thus, they can be able to express their emotions and thoughts without doing away with those of others.
