Critical reflection of your growth and development during your practicum experience in a clinical setting has the benefit of helping you to identify opportunities for improvement in your clinical skills, while also recognizing your strengths and successes.

- Refer to your Patient Log in Meditrek, and consider the patient activities you have experienced in your practicum experience. Reflect on your observations and experiences.

THREE MOST CHALLENGING PATIENTS:
PATIENT 1:
The patient is a 36-year-old female who presents for a follow-up to assess for cocaine and fentanyl abuse, bipolar, and depression. The patient came with cocaine overdose and 911 was called. The patient was asked to sit on the floor. She was agitated, restless, her anxiety level was high, she had excessive sweating and severe headache. She was given a shot of naloxone. The patient became stable and she was assessed. The patient endorses addiction, poor sleep, poor appetite, low energy, poor concentration, and sad mood. The patient appears shabby and confused. The patient denies SI/HI/SIB. The patient is referred to a detox/recovery program. The patient is to continue with Prazosin.
PATIENT 2:
The patient is a 75 old female who presents for a follow-up to assess for dissociative disorder, anxiety disorder, and depression. The patient states “I do not seem to remember things I plan for even my hospital appointments and some personal information.” The patient feels disconnected from herself and the world around her. The patient endorses feeling sad and depressed. She acknowledges crying spells, low energy, poor sleep, and poor appetite. She is easily irritated. The patient eates her anxiety at 7 out of 10 (10 being severe) and mood at 5 out of 10 (10 being best). PATIENT 3:
The patient is a 41-year-old female who presents for a follow-up to assess for anxiety disorder. schizoaffective disorder, and sleep disorder. The patient rates her anxiety at 7 out of 10(10 being severe) and her mood at 6 out of 10(10 being best). The patient endorses crying spells, poor sleep, poor appetite, and low energy. The patient denies SI/HI/SIB. The patient is to continue with Psychotherapy and Sertraline 25 mg PO daily. The patient was sleeping most of the interview and she was being waken up most of the time to answer questions. This made the assessment very difficult.
